Soccer Stars Club LLC – Participant Agreement
By registering my child for Soccer Stars Club LLC programs, I acknowledge and agree to the following:1. Release of Liability / Assumption of Risk
- I understand that participating in soccer activities involves inherent risks, including the possibility of injury.
- I confirm my child is in good health and able to safely participate.
- I accept all risks associated with my child’s participation and will immediately discontinue their participation if I believe any conditions are unsafe.
- I release Soccer Stars Club LLC, its coaches, staff, volunteers, and facility partners, including the owners and lessors of any premises where programs take place, from any claims or liability for injuries, damages, or losses that may occur, except where prohibited by law.
- I agree to indemnify, defend, and hold harmless Soccer Stars Club LLC, its coaches, staff, volunteers, and facility partners, including the owners and lessors of any premises where programs take place, from any claims, liabilities, damages, or costs, including attorney fees, arising from my child’s participation or from any claims made by anyone on my child’s behalf.

- I understand that Soccer Stars Club LLC may occasionally take photographs or videos during classes or events.
- These images may be shared with parents of participants, used for marketing purposes, and posted on social media accounts managed by Soccer Stars Club LLC.
- If I do not want my child’s image to be used, I will notify Soccer Stars Club LLC in writing or via the registration form before my child participates.3. Program Fees & Refund Terms
- I understand that all fees are due at registration and that Soccer Stars Club LLC does not provide refunds once a program or season has started; all sales are final.4. Emergency / Medical Authorization
- In the event of an emergency, I authorize Soccer Stars Club LLC and its staff to secure medical treatment for my child if I cannot be reached immediately.
- I understand that I am responsible for all costs associated with such care.
- I agree to provide accurate medical information, including allergies, medications, and relevant health conditions, to the program staff.5. Governing Law and Severability
- This agreement is governed by the laws of the State of Florida.
- If any provision of this agreement is found to be unenforceable, the remaining provisions shall remain in full force and effect.
